HOW TO FILL OUT THE SRVA FORM ONLINE

Go to www.srva.org ....click on SRVA ONLINE....on the next page click on SRVA ONLINE again....then click on INDIVIDUALS.....

If you did not play on any Southern Region team last year...click on "Add New Indv Info"...this will bring up the information screen....complete everything in YELLOW (PLAYER INFORMATION)....make sure you enter FEMALE...JUNIOR...and the player's grade in school....add a password....you do NOT need to provide the player's SS#.....then hit ADD....and another line should appear that says "Generate Individual Registration Form"....click on that and print a copy....bring this with you to tryouts.

If you played for a Southern Region team (Choo Choo, DIG, A-5, IMPACT, NASA, etc) when you click on INDIVIDUALS.....you can enter the last name of the player and the player's birth date and it will bring up a line with your player's name and will ask for a password....the password is your player's birth date using the following format xx-xx-xxxx put that in, and hit SELECT.....your players information should appear....double check the info....make sure the grade in school is correct and then put in the password (I found using the birth date in the xx-xx-xxxx format to be the easiest)....hit UPDATE, and the click on "Generate Individual Registration".....print the copy and bring it with you.

Players and Parents are both required to sign the membership form too.
